The care manager will be responsible for organizing care, coordinating with providers, and effectively utilizing the available resources. Strong communication, writing, and assessment skills are required.
Fundamentals/Summary of the Position:.
The Case Manager is responsible for conducting in-person visits with enrolled members in their homes while using thorough assessment tools.
The case manager is in charge of coordinating and collaborating on care along with the member/authorized representative, PCP, and any other members of the care team. The case manager represents the participants, schedules and attends interdisciplinary meetings, and advocates on their behalf to ensure a proper and safe discharge with the appropriate services in place. With the member and the care team, the case manager creates a care plan and cost-effectively approves services. The care manager must accurately and promptly record information in the member's electronic health record. To perform this role, a case manager needs to be able to think critically and solve problems. The case manager will also be expected to mentor new hires once they have a handle on their job responsibilities to some extent.

The base hourly rate or base annual full-time salary for all positions in the job grade under which this position is classified is represented by this pay range. The actual base salary offered will be based on a number of factors, including the candidate's background, level of education, geographic location, and other relevant factors. This position is also qualified for a CVS Health bonus, commission, or short-term incentive program in addition to the previously mentioned base pay range.
