Aetna Better Health is hiring for numerous positions across the state of Kentucky and its regions. The Case Management Coordinator (CMC) supports the case management process by using critical thinking and professional judgment to facilitate and maintain improved healthcare outcomes for members. Through the use of care management tools and resources, this is accomplished by offering advocacy, collaboration coordination, support, and education to members.
This is a telework position that requires regional in-state travel 80–90% of the time. A qualified applicant must possess reliable transportation. Additionally, it is anticipated that you will visit the Louisville office for conferences and training. Those in charge of this position will be Fulton, Hickman, Carlisle, Ballard, Graves, McCracken, Calloway, Marshall, Livingston, Lyon, Trigg, Crittenden, Caldwell, Hopkins, Christian, Muhlenberg, and Todd. Candidates who meet the requirements may reside in or close to the designated area.
It is essential to be able to work outside of the typical 8 a.m. to 5 p.m. CST Monday through Friday hours. We occasionally have to work into the night to accommodate the needs of families and children.

This pay range represents the Base Hourly Rate or Base Annual Full-Time Salary for all Positions in the Job Grade under which this Position is Classified. The actual base salary offered will depend on a number of factors, including experience, education, geography, and other relevant elements. This position is additionally qualified for a CVS Health bonus, commission, or short-term incentive program in addition to the aforementioned base pay range.

Obligatory conditions.
A minimum of six months' experience in the juvenile justice and foster care systems, adoption support, behavioral health service provision, trauma-informed care, ACEs, crisis intervention, or evidence-based practices that are pertinent to the SKY populations in Kentucky is required.
The candidate must reside in one of the following Kentucky counties: Fulton, Hickman, Carlisle, Ballard, McCracken, Graves, Calloway, Marshall, Livingston, Crittenden, Lyon, Trigg, Caldwell, Hopkins, Christian, Muhlenberg, or Todd, or an area close to those counties.
